Personal information of 747 Legacy Clinic Mount Hood (Mont.) patients stolen

What happened? Legacy Clinic Mount Hood has reported information on 747 patients and $13,000 missing.

What types of personal information? The stolen information may include names, credit card numbers, dates of birth, dates of service and Legacy account number.

What was the response? The clinic is in the process of contracting every patient affected by the thefts and will provide up to $25,000 in identity theft service and credit monitoring.

Details: The stolen information affects patients who received care between January 2006 and February 2007.

The thefts were discovered by a clinic doctor and may involve a former employee.
